The character is simply the son of Carnage, created from twisted thinking. The Doppelganger is a Spider-Man with no moral code, driven by a killer instinct and with an appearance closer to a spider.
Carnage begins infecting as many people as possible with his symbiote virus. The one who discovers the case is Detective Misty Knight in Doverton city.
There, she discovers a cult of symbiote gods, among them Knull and Carnage himself. To make matters worse, it is revealed that Shriek, considered the mother of the Doppelganger, is also alive.
The animalistic clone of Spider-Man enters the story as a sort of soldier of the villains. The Doppelganger is behind a series of murders in the city. The goal is to spread the Carnage cult.
